NON-N.H.S. MEDICALS AND PRIVATE FEES

Medical examinations for H.G.V., P.S.V., Insurance and Employment etc. can be arranged by appointment. This sort of work, including claim forms for holiday cancellations and medical insurance are not part of your doctor's N.H.S. duties and therefore a fee within the range recommended by the British Medical Association is payable, details of charges are available at reception.

MEDICAL CERTIFICATES

For absence from work through sickness, self-certification is currently valid for the first week. The certificate (form SC1) is available from your employer, the post office, or the surgery reception. We do not issue an N.H.S. Doctor's certificate for the first seven days of sickness.

If a private certificate is required to cover the first week of absence from work, a fee is charged. Following this, certificates can be obtained as part of a consultation with your doctor. If you are under the care of a hospital doctor you may ask them to issue your certificate for an inpatient stay or an outpatient appointment.

COMPUTER

The surgery is fully computerised and is reducing its use of additional paper records more and more. We have pioneered computer links with the Health Authority have joined the N.H.S. net and are involved in other projects. The practice is registered under the Data Protection Act 1998, which enables you to view your record.

F2 DOCTORS (Dr Lisa Moore August 2011)

We are fortunate to have F2 doctors regularly gaining experience at the practice from Liverpool University and we would be grateful for your help with their training.

We will inform you if the F2 doctors are sitting in with your doctor but you may of course ask
them to leave if you are uncomfortable with this. During the end of their rotation at the practice, the F2 doctors will be seeing patients on their own, once again we will inform you of who you will be seen by.

PATIENT CONFIDENTIALITY

Occasionally third parties such as auditors and researchers request to see patient notes. All individuals with access to your data have a professional and / or contractual duty of confidentiality. If you do not want your records accessible by anyone external to the practice please contact us and let us know.

DISABLED PROVISION

Full wheelchair access and disabled toilet facilities are provided. The surgery has a portable loop system for the hard of hearing.
